Manuscript Format
- Submit your manuscript electronically as a Microsoft Word file (extension ".doc") or a rich text format file (extension ".rtf") if you use a different word processing program. Submit all figures as separate graphics files; do not place in Word documents. (See below for graphics file format guidelines.)
- Submit each chapter as a separate file that has one-inch margins, is double-spaced, and is in twelve-point Times New Roman.
- Please keep formatting to a minimum: Use italics instead of underlining. Be consistent in formatting headings to indicate their levels (e.g., bold for first-level, italic for second-level, run-in italic for third-level), or simply label heads in brackets (e.g.: [A]This Is an A-Head). Use a consistent convention throughout your manuscript.
- References should be footnotes or endnotes (use the "Insert Footnote" feature in MS Word or the equivalent in your software so that notes are linked to the text and the numbering is automatic).
- Where a figure should be inserted in the layout, please type on a separate line in bold: [Insert Figure 1.1.].
- On the line following "[Insert Figure . . .]," provide the title of the figure (e.g., Figure 1.1: Coronary Venography) and the credit line, using the exact wording dictated by the copyright holder.
- Cardiotext recommends using the Table function in Microsoft Word to create tables. Alternatively, use tabs to separate columns and paragraph returns to separate rows. Give each table an appropriate number and a title (e.g., Table 5.1: Intraoperative Evaluation). If a table is reprinted from a previously published work, provide the credit line, using the exact wording dictated by the copyright holder, on a separate line below the table.
Graphics
File Formats
TIFF and EPS files are preferred.
JPEGs are also acceptable.
Quality (Resolution) and Appearance
All graphics should be submitted in the highest resolution possible, measured in dots per inch (dpi), at 100% of the size they will be printed in.
Photographs and illustrations: 300 dpi
Line drawings: 1200 dpi
Machine images: highest resolution the generating machine can produce
Submit only raw image files; do not place them in Word or PowerPoint documents.
Labels applied in Word or PowerPoint are helpful as directions for the designer, but labeled images are to be avoided as files for actual use.
Crop images (if necessary) before submitting them, or provide cropping instructions with the figure information in the manuscript.
Indicate if images should appear in color.
If placement of labels is unavoidable, please limit use only to identify specific sections of the image. Lablels should never be used as references, headers or call-outs from the text.
Distinct image files should not be combined into one image.
File Names and Organization
The manuscript should include references to the figures (as well as captions and credit lines) so it is clear where each image is to be placed.
File names should correspond to figure numbers in the manuscript (e.g., Figure 5.1.tif).

Bibliographic Style
- Any standard bibliographic style (e.g., AMA) is acceptable.
- Please let the publisher know which bibliographic style is used in your book. Editing will proceed on the basis of enforcing the consistency of the predominant style used in the manuscript.
